To shift pediatric care from reactive treatment to proactive health management, this study proposes a purpose-driven semantic framework based on the Data, Information, Knowledge, Wisdom, and Purpose (DIKWP) model. By leveraging artificial intelligence, the framework transforms raw health data into actionable wisdom aligned with each child's specific health goals. We present a system architecture centered on a DIKWP semantic graph and a cognitive reasoning engine to enable early interventions, personalized recommendations, and improved clinician-family collaboration. The framework is formalized using a semantic mathematical model, including a bidirectional cognitive mapping to bridge communication gaps between patients and clinicians. A use case demonstrates the system's ability to anticipate health issues and align decisions with patient-specific purposes. This approach introduces a paradigm for anticipatory and purpose-oriented pediatric care, aiming to improve health outcomes by making healthcare safer, more effective, and transparent.
